We have been performing and guiding patients through Immigration Medical Exams by a civil surgeon for over twenty years. We specialize in completing the I-693, Report of Medical Examination and Vaccination Record for the USCIS (U.S. Citizenship and Immigration Service).

What to bring with you to your exam:
Please bring any past vaccine records and a government-issued photo ID with you. Please bring an interpreter if needed.

What to expect for your exam:
After you check in, a clinical assistant will have you fill out the first page of the I-693 form and take your blood pressure and pulse. We will review your medical history and perform an exam. We will then review with you which vaccines and labs are needed. Typically these vaccines and labs will be done on the same day of your visit. Usually you will need to return in 2-3 days to have your TB test (PPD) checked. If you live far away or can’t return, we can do a routine blood test instead of the PPD. Total time spent in the office the first day is usually one hour.

Typically a full Immigration Medical Exam, performed by a civil surgeon, and processing the paperwork costs $200 without any vaccines or labs. Most people do need updated vaccines and labs. This will add to the cost of the exam. Payment is by cash or credit card. If you are a refugee or already had your Immigration Medical Exam done in another country, you will most likely only need the vaccine supplement form filled out. This costs $130 plus the cost of any vaccines needed.

Getting your forms:
You can choose to pick up your completed form at our office or we will mail it to you. You can open the yellow envelope. Inside you will find a white envelope and a copy of the I-693 form. Do not open the white envelope. That should go to USCIS or your lawyer.

Our goal is to make this process as easy as possible for you. Please let us know if you have any special requests. If you have more than one person that needs to get an exam, let us know so we can book you in back-to-back appointments.

We can see you even if you live in a variety of states, including Maine, New Hampshire, Massachusetts, Vermont, Connecticut, Rhode Island or New York. If you are in a rush, please let us know so we can see if we can fit you in the same day.

We are conveniently located right off of 295 in Portland, Maine, 84 Marginal Way, 7th floor, but we see many patients from all over Maine including Augusta, Lewiston, Bangor, Falmouth, Scarborough, Yarmouth, South Portland, Waterville and Sanford. We also see a lot of patients from New Hampshire including North Conway, Portsmouth, Nashua, Manchester and Concord. We have a free covered parking garage.

All of your medical questions will be answered during your exam. However, feel free to call us if you have more questions or if you would like to make an appointment. Our goal is to get your exam done as quickly as possible for you and with the least cost. We typically schedule exams every other Tuesday afternoon, but arrangements can be made if needed sooner.
