InterMed patient COVID vaccination scheduling

Patients 16 and older are eligible to receive the COVID-19 vaccine at InterMed.

  • Patients under the age of 18 must be accompanied by a parent or guardian to receive the vaccine or must bring a consent form signed by a parent/guardian.

Vaccination is by appointment. We also offer limited walk-in appointments.

What you need to know

  • You should not be vaccinated if you have received any other immunization within the last 14 days. The CDC recommends waiting until 14 days have elapsed since your last shot to receive the COVID-19 vaccine.
  • It is your responsibility to discuss any vaccine concerns with your primary care provider before vaccination. For information about the COIVD-19 vaccines, refer to the Pfizer COVID-19 Vaccine Fact Sheet or the Moderna COVID-19 Vaccine Fact Sheet. Vaccine preferences cannot be accommodated.
  • Your visit will require 30 minutes, including a mandatory 15-minute observation period.
  • Second dose appointments will be scheduled in-person during your first dose appointment. The Pfizer vaccine requires a second dose, given 21 days apart. The Moderna vaccine requires a second dose, given 28 days apart. First and second doses are both administered at the InterMed Vaccine Clinic.

Bring to your appointment

  • Signed Consent & Administration Form. Download, print, and sign prior to arrival.
  • Driver’s License or other form of government issued photo identification.
  • Insurance Card and red, white, and blue Traditional Medicare ID, if applicable. The vaccine is available at no cost to you. If you have insurance, InterMed will bill the administration fee to your insurance provider. If you do not have insurance, you will not be billed for the vaccine or its administration.
  • Your EpiPen, if you typically carry one.

  • Walk-in appointments are first come, first served
  • Walk-in hours are 8:30 am – 3:30 pm
  • Availability is limited.
