Patients 16+ are eligible for vaccine

. Posted in Coronavirus

Dear Patient,

Tomorrow, April 7, the State expands its COVID-19 vaccine eligibility guidelines to include all Mainers 16 years and older. This message contains instructions on how to schedule an appointment.

Please be advised that vaccine is delivered to InterMed on a weekly basis and therefore we are only able to offer appointments a few days in advance.

To schedule your vaccination appointment, you may now book online at www.intermed.com/covidvaccine

Please note that the vaccines are given by appointment only, and that we have a limited number of appointments each day. Additional appointments are released periodically, so check back if you don’t find something that fits your schedule.

Also, please do not schedule an appointment if you have received any other immunization within the last 14 days. The CDC recommends waiting until 14 days have elapsed since your last shot to receive the COIVD-19 vaccine. Patients under the age of 18 must be accompanied by a parent or guardian to receive the vaccine or must bring a consent form signed by a parent/guardian.

Our Vaccine Clinic is located at 225 Gorham Road in South Portland, near the Maine Mall. You’ll find directions, as well as a Consent and Authorization Form at www.intermed.com/covidvaccine. Please print, sign, and bring a copy of the Consent and Authorization Form to your appointment. Please check with your doctor if you have any concerns about receiving the vaccine before coming to your appointment.

We ask that you also bring:

  • Your driver’s license or other form of government issued photo identification
  • Your insurance card or Traditional Medicare ID (the red, white, and blue card)
  • Your EpiPen, if you typically carry one.

Your appointment will require 30 minutes, including a mandatory 15-minute observation period.

  • Do not miss your appointment. Missing your appointment will delay vaccination.
  • Please arrive no earlier than 5 minutes prior to your scheduled appointment. Anyone arriving more than 10 minutes late will need to reschedule.

The COVID vaccine requires two shots, given either 21 or 28 days apart, depending upon which vaccine you receive. We will schedule your second shot during your visit.

We understand that there are many options in the community to receive the vaccine and access varies by site. The State of Maine offers a comprehensive list of vaccination sites and VaccineFinder.org is an additional resource for those in search of a specific brand of vaccine. If you receive a vaccine elsewhere, please let your provider know so that we can update your records..

We are pleased to be able to offer you the COVID vaccine and look forward to seeing you at our Clinic.

Please stay safe and well,

Dan Loiselle, MD, Chief Medical Officer, and the entire InterMed Care Team
