Patients of our Pediatric Department under age 13: Parents, please call (207) 847-5395 to sign up. MyInterMed Patient Portal accounts are not available for children ages 13-17.

Adult patients (18 years and older): Please complete the form below. We will contact you by phone during our normal business hours to confirm your information and set up your account.

Adult Patients – important information

  • Your username will be your email address.
  • Email addresses must be unique for each patient.
  • Shared or work email accounts should not be used, as you will receive messages regarding your confidential health record.
